Here is a checklist of the documents and information that parents need to provide when registering for daycare when spots are available:
- Birth Certificate for all children enrolling and not enrolling
- Yellow immunization card with up-to-date vaccines
- Child’s health insurance card
- Up-to-date medical documentation (must include an office stamp)
- Up-to-date dental records for children aged 1 year and older (must include an office stamp)
Proofs of Address (Provide at least 2 of the following):
- Current utility bills (e.g., light, gas)
- Current lease agreement
- Health insurance letter
- Medical bills
- SNAP (Supplemental Nutrition Assistance Program) approval letter
Proof of Income (Provide at least one of the following):
- W-2 forms
- Tax returns
- Paystubs
- Employment verification form (notarized)
- SNAP budget letter
Escort Form:
